•Initial Review: $100 for a 1 hour program per discipline requested (RN< SW< PT); $50 for each additional CE hour (please note that SW credit is good for LCPC as well)

•Repeat Program: $50 for each repeated program per discipline requested (RN< SW<pt); no="" matter="" length="" of="" program.="" 'repeat"="" references="" presenting="" the="" same="" topic="" on="" a="" different="" day="" or="" multiple="" events="" day.

• CCM: Altra Healthcare Consulting can apply for CCM credit on your behalf for additional charge of $125 for the initial event, $25 for each event after that for 1 year after the initial CCM certification. CCM credit must be applied for at least 4 weeks prior to the initial event.

Certification Period: Certified topics remain valid for 1 year from initial approval. Previously reviewed /certed topics need to be re-reviewed for content yearly.

Paperwork Required for Initial CE Review:

For ALL of your CE programs, prior to initial certification, we will need:

1.Title

2.Short Paragraph about the program

3.3 or 4 objectives to be evaluated by participants

4.Date, time, location and address of program

5.Speaker name, title, contact information (email and phone) and biography/CV

6.A copy of your promotional material-this can be a flyer, brochure, page from your website, etc.

Repeat Programs

Prior to each repeat of a currently certified program, you must submit:

1.the title of the program

2.Date, time, location and address of program

Post Program Requirements (required by state of IL for compliance)

After each program we will require:

1.Attendance list (provided)

2.Evaluations (provided)

LIVE EVENTS: Attendee Sign In and Evaluations need to be scanned and returned to Altra Heathcare Consulting within 48 hours post-event. Certificates will be provided to be handed out at the LIVE event. Certificates must be exchanged for completed evaluations (no exceptions).

VIRTUAL EVENTS: Altra Healthcare consulting offers ZOOM platform hosting (up to 100 attendees) and attendance /evaluation management for $25/event. For events with >100 attendees, there will be a $50 additional charge for the additional ZOOM license.

If you choose to use your own virtual platform, an attendee report (showing length of time on the meeting/attendee) is required before an evaluation link can be sent to your attendees. 

Once the evaluation is received from individual attendees, certificates will be sent from Altra Healthcare Consulting. The post-event evaluations are open for 14 days post event. Attendees who do not complete evaluations through the link provided within 14 days will forfeit credit for the event.  Invoices that remain outstanding 60 days or greater after certificate issuance will be subject to certification revocation for that specific event and attendees will be notified of withdrawal of CEU Certification
